For purposes of this strategic planning exercise, you will be assigned a number that will be used to identify your submission. This will keep your submission confidential, but allow the team to contact you if needed for this work. 

The list containing the names and numbers will be kept by the team leader and rector only. It will not be available to any other person.

The information and opinions that you submit will be used by the Strategic Planning Team members only to complete their work on this project. 

The information that you submit will be held securely and made accessible only to the team for work on this project. 

When the work on this plan is complete, the information that you submit will be securely stored for a maximum of five years, and then securely destroyed. 

When you complete the survey, check the Consent Agreement box to indicate that you understand the use and retention of your submission information and agree to the terms outlined above.
